Clinical Analysis of 500 consecutive Laparoscopic Cholecystectomies


Abstract
Laparoscopic cholecystectomy is a common operation and has become standarized. It requires the additional skill of a trained surgeon, and its safe performance seems to be related to proper training and experience.
The results of laparoscopic cholecystectomy, compared with those of open cholecystectomy, are much better with respect to the patients's well-being, the short operative time, the absence of physical or cosmetic damage, and the quick return to
normal
physical and professional activities.
We performed 500 consecutive laparoscopic cholecystectomies at the department of general surgery of Taejon Eulji General Hospital from 9th january 1992 to 5th Feburay 1995.
@ES Outcome cata from the 500 patients have been reviewed.
@EN 1) The age range of the patients was from 18 to 81 years with average being 53.2 years. There were 308 females and 192 males.
2) From the time of the onset of symptomes to coming to our hospital, the majority(82.4%) of admitted patients came in a years length of time.
3) Patients who were done Preoperative Endoscopic retrograde cholangiography and Endoscopic shphincterotomy were 121 cases(24.2%) and 44 cases(8.7%).
4) Majority(83.0%) of patients have mild to moderate inflammation and adhesion.
5) The removed stones by laparoscotic cholecystectomy vary in size of number.
6) The basic types of removed gallstones were cholesterol stones(12.0%), black pigment stones(46.7%) and brown prgment stones(41.3%).
7) Mean operation time was 46 minutes.
8) In 85 Patients, drains were placed following laparoscopic cholecystectomy
9) Operative cholangiogram was attempted in 42 patients and successfully in 38 cases.
10) Among the 500 patients, eight complications occured in twenty-four patients : common bile duct injury(£ª2), subcutaneous emphysema(£ª5), abdominal wall hemartoma(£ª2), bile leakage, Wound infection(6£ª), pulmonary complication(£ª5), remained
common
bile duct stone(£ª2), puncture site bleeding(£ª1).
11) Postoperative oral intake begins within 24 hours mostly.
12) The mean length of hospital stay was 2.46 days.
13) the common microoganisms of bile culture were escheria cli, Pseudomonas, Streptococcus, and Klebsiella.
